• Coverage varies depending on your specific plan, and not all policies include chiropractic benefits. Patient Responsibility of copays, co-insurance, and deductibles will also differ from plan to plan.

• If you have a PPO plan, you can typically schedule chiropractic care without a referral.

• For HMO plans, a referral from your primary care physician (PCP) is required in order for us to bill your insurance.

Insurance typically covers chiropractic adjustments to the spine only.

Manual therapy and extremity care are optional add-on services available for an additional $60.
